If you are interested in receiving a care package for you Wobbler, please submit the below application. Captain's Salute care packages are better suited for dogs with a recent diagnosis. All applications will be reviewed and verified by our team.  Please note that a diagnosis by MRI and/or CT is required to be eligible for a care package - we will verify the imaging results with your pet's neurologist. You may opt to email your confirmed diagnosis to captainssalute@gmail.com to save time (if you are outside of the United States, you MUST submit the MRI and/or CT results from your neurologist to us via email). Once your application and results are confirmed, you will receive a short survey about your Wobblers needs in order to customize your care package.
